Just like you I also move my mouth while reading. I find that it really helps in boosting my concentration.

I do it here and there. A voice in the back of my mind keeps telling me not to do it, as if I were a toddler who's just learning to read. "Don't do it! You'll mess up your learning and never read properly!" so I kind of try to avoid it. Hopefully I'm just being silly about it.

When I get into a certain "zone" (you know the one), this stops as I get immersed in it.

I don't move my mouth when am reading unless when skimming though a section.

It depends on the mood, sometimes my brain cannot focus on the written words and make sense of it until I whisper, sometimes at late night (2am) I must utter the words in a low-voice, while sometimes my brain prefers to silently read, such as when I'm returning from a jogging session in the morning or after a cold shower, it really depends on the situation I find myself in.

I read book in many ways. I move my mouth while reading or just read with a closed mouth. Also, sometimes I whisper while reading a book.

Not often. When I'm having trouble catching up with the book's pace, or understanding a particular section, or when I find a very good description, only then do I mouth it or mumble it.

I don't usually move my lips when I read but I have caught myself doing it on occasion. I think it may happen when I am trying to understand a difficult part. I don't know if it helps or not!
